Initiative #1: Type 1 Diabetes leading the way to fix rural and poverty related health
Goal: create a pilot in Florida that will use the Heroic Diabetes platform to show the data and funding model to scale up a national education program over the first 3 years. This will become the model that is used for all kids with chronic conditions.
Target Audience: K-6 students in Collier County and the adults that they influence. Eventually will expand to K-12 and college students nationally.
Partners: Heroic Diabetes, Help a Diabetic Child, Core Health Partners, Collier County School District, University of Florida, Florida Dept of Health.
Funding: private and public grants, and after 1 year will be replaced by the VBC billing for Diabetes Self Managed Education and Diabetes Prevention Program.


The Initiative Requirements:
1. Must be scalable to all of Florida and the whole country (K-12, college and/or adults) over the course of 3 years
2. Must have data indicating that it can decrease existing healthcare costs and generate a minimum of 100x ROI
3. Providers must be able to offer a "guaranteed results" promise to the project or money refunded
4. Can be funded initially by grants or investment, but must eventually be supported by VBC billing
